Medical insurance is provided through Kairos. Kairos medical plans use the UnitedHealthCare (UHC) Choice Plus network and MaxorPlus network for prescription drugs. Medical claims are administered by UMR. For SY 2025-2026 there are three medical plan options.
It is up to you to determine which plan is the best fit for yourself and/or your family.
HDHP $1600, $3,300 Plan Summary of Benefits Coverage 2025-2026
*Generally, you must pay all of the costs from providers up to the deductible amount before the HDHP plan begins to pay. If you have other family members on the policy, the overall family deductible must be met before the plan begins to pay.
**If you are approaching Medicare eligibility, we recommend reaching out to the Pima Council on Aging (PCOA) or Medicare to learn more about the interactions between high deductible healthcare plans, health savings accounts, and Medicare.
Employees enrolled in the HDHP plan will have to pay for Teladoc services beginning 7/1/2025. Please see the Benefits Guide for more information.
Some preventive care services are covered at 100% by all three plans.
Employees enrolled in the Gold and Silver Medical plans have FREE access to use Teladoc from the comfort of your home. This is a District-Paid benefit that provides phone or online access to a doctor 24/7/365. Employees enrolled in the HDHP plan will have to pay for Teladoc services beginning 7/1/2025. To use Teladoc, you can reach a U.S Board-certified doctor on the Teladoc app or you can call at: 1-800-835-2362.
NOTE: You do not need your medical ID to access your Teladoc benefits. However, if you've never used Teladoc, you must register with Teladoc before using their services. When calling, please provide your name and inform them that you are an employee with Amphitheater Public Schools. If they have trouble accessing your Teladoc account, please reach out to the Benefits office for assistance.
